Patient Forms may be completed electronically through our online portal. Once you schedule your consultation, a link will be provided to you.

If you are not able to complete the portal paperwork, please review our Privacy Practices and print out and complete the New Patient Forms below. Please call our office if you have questions regarding our Privacy Practices or your paperwork.

Privacy Practices

Dr. Warren Schutte Plastic surgeon offers free cosmetic surgery consultations in our Loveland, Colorado location. Please note that we no longer take insurance. If for any reason you need to cancel your consultation with us, please provide us with at least a 24 hour notification. Not showing up for your consultation or not giving us a 24 hour notice may result in a future charge.
